4. DECLARATION
By submitting this form I hereby: *

i) confirm that I meet the eligibility criteria set out in section 1 above;
ii) agree to become a member of Create Gloucestershire Limited (the "Company");
iii) agree to abide by the Company's articles of association;
iv) agree to pay to the Company an amount not exceeding £1.00 if the Company is wound up while I am a member or for up to 12 months after I have ceased to be a member of the Company; and
v) confirm that my name can be entered into the register of members of the Company.

FURTHER INFORMATION

a. Please note that the Company reserves the right to (but shall be under no obligation to do so) request further information or evidence of information before considering your application.

b. You will be informed of the outcome of your application, however, the Company shall be under no obligation whatsoever to provide reasons.

c. The Company has the authority to apply a membership fee.  However, it is not anticipated that new members would pay a fee for the first 12 months of their membership.

PLEASE NOTE: If you do become a member you can be removed from membership if you do not comply with the Company's articles of association, a copy of which is available on request.
